چکیده

In current screening test for Down’s syndrome (namely maternal serum alpha-fetoprotein-AFPhuman chronic gonadotropinhCG-, unconjugated estriol plus maternal age) an elevated level of hCG is an important predictor of fetal aneuploidy (1). The positive predictive value of the multiple marker screening test is approximately 4.3% (2). Besides Down’s syndrome, triploidy, trophoblastic diseases, and multiple gestations have been associated with elevated hCG levels (3). In the majority of cases the cause of the elevated maternal hCG levels is not known. hCG is produced by placental trophoblasts and excreted directly into the maternal circulation, and its is possible that abnormal levels are an early sign of placental dysfunction.

Screening models using multiple markers for early detection of late-onset preeclampsia in low-risk pregnancy

BACKGROUND Our primary objective was to establish a cutoff value for the soluble fms-like tyrosine kinase 1(sFlt-1)/placental growth factor (PlGF) ratio measured using the Elecsys assay to predict late-onset preeclampsia in low-risk pregnancies. Our secondary objective was to evaluate the ability of combination models using Elecsys data, second trimester uterine artery (UtA) Doppler ultrasonogr...

Advances in prenatal screening for Down syndrome: II first trimester testing, integrated testing, and future directions.

BACKGROUND The acceptability of prenatal screening and diagnosis of Down syndrome is dependent, in part, on the gestational age at which the testing is offered. First trimester screening could be advantageous if it has sufficient efficacy and can be effectively delivered. ISSUES Two first trimester maternal serum screening markers, pregnancy-associated plasma protein-A (PAPP-A) and free beta-...

Hyperreactio Luteinalis with early-onset HELLP syndrome: A case report

Introduction: Hyperreactio Luteinalis (HL) is a rare benign condition in pregnancy which is characterized by bilaterally multicystic ovarian enlargement containing theca lutein cysts caused by increased production of hCG. HL is mostly associated with hydatidiform mole and multiple pregnancies. Correspondence: Azin Alavi, ...

Mid-Trimester Maternal Serum hCG and Alpha Fetal Protein Levels: Clinical Significance and Prediction of Adverse Pregnancy Outcome

CONTEXT Maternal serum human Chorionic Gonadotropin (hCG) and Alpha Fetal Protein (AFP) were originally introduced to detect trisomy 21 and neural tube defects. However, in the absence of aneuploidy or neural tube defects, mid-trimester maternal serum hCG and/or maternal serum AFP associated with adverse pregnancy outcomes. Pregnancies with unexplained mid-trimester elevation in maternal serum ...

Dimeric inhibin A as a marker for Down's syndrome in early pregnancy.

BACKGROUND In screening for Down's syndrome in the second trimester of pregnancy, the concentrations of alpha-fetoprotein, the beta subunit of human chorionic gonadotropin, and intact human chorionic gonadotropin in material serum are widely used markers. We investigated a new marker, dimeric inhibin A, and compared its predictive value with that of the established markers. METHODS Serum samp...
